EPF Form 10C

Employees’ Provident Fund (EPF) is a scheme wherein both employee and employee make certain contributions towards EPF and EPS accounts. When you decide to change your job, you can either withdraw the pension amount or transfer it to the new one by submitting the EPS Scheme Certificate. This certificate shows the service details of the employee and the details of the family members who are eligible to claim a pension in case of the death of the member. However, the accumulated pension can be withdrawn by the members by using Form 10C after 180 days of continuous service and before completion of 10 years of the service period.

Format of Form 10C

The form consists of 4 pages. The format is given below:





Contents of Form 10C

Form 10C contains the following sections that an individual need to fill at the time of applying for pension withdrawal:

  • Name of the Claimant
  • Date of Birth
  • Father’s/Husband’s Name (in case of married women)
  • Name and Address of your Establishment where you were last employed
  • Establishment Details (Office code, establishment code no. and account number)
  • Date of Joining the Establishment
  • Date and reason for leaving the service
  • Your willingness to accept Scheme Certificate instead of withdrawal benefits
  • Details of Family and Nominees (Spouse & Children & Nominee) – Name, date of birth, relationship with the member, and name of the guardian (for minors)
  • Details of the member and nominee (in case of death of the member after attaining the age of 58 years without filing the claim)
  • Remittance mode (postal order, cheque, or savings account)
  • Whether you are applying to avail pension under EPS, 1995
  • Signature of the employee and employer (along with the organization’s seal)
  • Advance Stamp Receipt – settlement receipt for pension in the savings account
  • Employee Scheme Certificate which is to be filled by the PF Commissioner’s office

How to Fill Form 10C Online?

Step 1: Firstly, log in to the EPF Member Portal by using your UAN and password.


Step 2: After that, select “Claim (Form – 31, 19 and 10C)” under the “Online Services” menu.


Step 3: Now, enter the last 4 digits of your bank account number and click on “Verify”.


Step 4: Sign the “Certificate of Undertaking” and click on “Yes” to agree to the terms and conditions.


Step 5: Now, select the “Only Pension Withdrawal (Form 10C)” option in the “I want to apply for” section.


Step 6: Enter your complete address, put a tick on the disclaimer, and click on the “Get Aadhaar OTP”.


Step 7: An OTP will be sent to your mobile number registered with Aadhaar (UIDAI). Enter this OTP and then click on “Validate OTP and Submit Claim Form”.


Step 8: Your pension claim form will be submitted, and the fund will be disbursed in your bank account after verification by the EPFO.

Eligibility to apply for Form 10C

  • The members of EPS who fulfil the below criteria can apply for the Form 10C:
  • If have left employment before the completion of 10 years and have attained the age of 58 years before completing 10 years of service.
  • An individual who has completed 10 years of service, but have not attained 50 years of age, or a member who is between 50-58 years of age and is not willing to settle with a reduced pension.
  • Nominee/family of the member who died before 10 years of service whose age was more than 58 years at the time of death.

Documents required to be attached with Form 10C

  • A copy of cancelled or blank cheque
  • Date of birth certificates of children while applying for scheme certificate
  • Succession certificate for legal heirs of member
  • Rs. 1 stamp is required in the case of applying for withdrawal benefit through a bank

Instructions for Filling Form 10C

  • Always write the names in capital letters only
  • The date of birth should be exact
  • Make sure there is no cutting or overwriting in the form, and if there are corrections, the same must be attested
  • The PF account number consists of region code (2 alphabets), office code (3 alphabets), code number (maximum 7 digits), extension (sub- code), and the account number (less than or equal to 7 digits)
  • In the form, serial no 9 should be filled if applying for a scheme certificate, while serial number 11 can be left blank
  • Serial no. 11 is required to be filled while applying for a withdrawal certificate, where Serial No. 9 can be left blank
  • Serial no. 9 needs to be filled along with the details of the nominee/family/legal heir, in the case of the member’s death. While Serial No. 10 and 11 are required to be filled by the respective legal heir/nominee/family member
  • If the member is drawing pension under the EPS scheme 1995, Serial number 12 needs to be filled

Attestation Procedures

  • The members need to attest to the EPF Form 10C by either the employer or both the employers and employees depending on the circumstance. The following cases are –
  • In case, an individual has applied using a form procured from a center; it needs to be attested by their previous establishment’s employer.
  • If the individual has downloaded the EPF Form 10C online, both the employers and employees need to attest it.
  • In case, the previous establishment has been closed, whose Authorized Signatory/ Employer is not available, in that case, you would need any one of the following authorized officials shall attest with his office seal such as-
  • Magistrate
  • Gazetted Officer
  • Post/ Sub-Postmaster
  • President of Village Union
  • President of the Village Panchayath when the Union Board is not present
  • Chairman/ Secretary/ Member of Municipal/ District Local Board
  • Member of Parliament/ Legislative Assembly
  • Member of Central Board of Trustees/ Regional Committee of Employee’s Provident Fund
  • Manager of the Bank in which Savings Bank account is maintained
  • Head of any recognized Educational Institution
