EPF Form 5

EPFO covers all the organizations that have 20 or more employees. These organizations need to provide EPF services to the eligible employees and contribute to their Employee Provident Fund, Employee Pension Scheme, and Employees Deposit Linked Insurance. For this purpose, they need to inform the EPFO about new joiner who are eligible for EPF services every month by filling Form 5 and submitting it to the regional EPF Commissioner.

Importance of Form 5

EPF Form 5 is extremely important for a new joiner who have not had previous experience with the employees’ provident fund. With the help of this form, new members can avail of the benefits offered under the provisions of EPF, without which their social welfare and security would be incomplete. It also helps the employer to keep a track of new employees and ensure that every member is covered under EPFO rules.


Components of EPF Form 5

EPF Form 5 contains the following sections that employers have to fill up and submit-

  • Month and year of the submission of the form
  • Name and address of the Establishment/Company
  • Code number of the Factory/Establishment
  • Details of the employee
  • Serial Number
  • Account Number
  • Name of the Employee
  • Father’s Name (Husband’s Name in case of married women)
  • Date of Birth
  • Gender
  • Date of joining the Fund
  • The total period of the previous service as on the date of joining the Fund (Enclose Scheme Certificate as applicable)
  • Remarks
  • Signature of the employer or any other authorized officer of the Factory/Establishment
  • Stamp of the Factory/Establishment
  • Date of filing the Form
  • The form should be signed and stamped by the employer along with the date of filing.

How to Fill EPF Form 5?

The employer is required to fill EPF Form 5 and mention all the details of the new employee. They can take the details from Form 2 (nomination details) and Form 11 (basic EPF details).
